You can use online services at the surgery to book an appointment, order a repeat prescription, get a sick certificate, get test results, ask about a non-urgent medical problem and view your medical record.
Which online service to use?
There are different online services for different tasks.
The NHS App
- Check your symptoms to get health advice;
- Find out what to do when you need help urgently;
- Book & manage appointments at the Surgery;
- Request repeat prescriptions;
- View your GP record securely;
- Register to be an organ donor; &
- Choose how the NHS uses your data.
For further details, please visit www.nhs.uk/nhsapp
or see this promotional video on YouTube: https://youtu.be/421wy0ND9Ls
The NHS App is really easy to use & it is an effective alternative to our current Patient Access Online Services system. It is available free to download for iOS from the App Store & for Android from Google Play.
Unlike our current Patient Access system, you can even prove your identity using the App itself without needing to bring any ID to the surgery. If you are already a user of Patient Access, you will be able to access exactly the same information on the NHS App as you currently do on Patient Access.
We would encourage all patients who are able to, to use the NHS App to book their appointments at the Surgery & to request their repeat prescriptions, as this frees up our administrative staff to deal with other matters. If you are having any problems using the NHS App, please click here to contact their team directly.
If you do not own a smartphone or tablet, you can still sign up for and use Patient Access instead.

Registering for online services
To register for online services, so you can book appointments, order medication and view your medical record, fill in the online access application form.
You can also download and complete the form and bring it into the practice or email the completed form along with two forms of ID (Passport, driving licence etc. and proof of address) to: swlicb.fairhillmedicalpractice@nhs.net
You will then need to bring photo ID and proof of your address to the surgery, where the receptionist will print your online access codes.
If you are a newly registered patient, we will have to wait for your medical record to arrive at the surgery before we can process your request to view your medical record.
Registering for someone else on their behalf
You can register for someone else on their behalf (proxy access) if you are a carer or relative.
If you wish to have your child to have online services to book appointments you can also be given proxy access.
To register on behalf of someone else, complete the online proxy access request form. You can also download the form and bring photo ID and proof of your address to the surgery, or you can email your ID to: swlicb.fairhillmedicalpractice@nhs.net
Proxy access needs to be authorised by a GP. The practice will phone or text you to confirm the request has been authorised.
