You only need a sick note if you are unable to work and are ill for longer than 7 calendar days.
You can request a sick / fit note by:
- Use an online form to request a sick / fit note or to renew a certificate which will normally be a phone call with our Nurse.
- Call the surgery to book a telephone appointment with our Nurse.
For sickness periods less than 7 days there is a charge for a private certificate as you sickness less than 7 days you don’t need a sicknote
